By submitting this application to the American Water Spaniel Club Rescue, I certify that I am at least 21 yours of age and that I will be the legal owner and ultimately responsible for the care and well-being of any dog I adopt from the American Water Spaniel Club Rescue. Any misrepresentation of the true facts in the application or in the adoption agreement will invalidate the agreement. I also agree to give my veterinarian and other references permission to disclose information to American Water Spaniel Club Rescue.
